1. ALW Application Support & Assisted Living Placement Guidance

The Assisted Living Waiver (ALW) is one of California’s most important programs for low-income seniors who require supportive housing. AXH Foundation provides culturally competent guidance to help families understand, navigate, and access ALW benefits.

Our services include:

  • Eligibility screening for ALW

  • Document preparation guidance

  • Explanation of the application and assessment process

  • Coordination with participating RCFE/Assisted Living providers

  • Transition planning and move-in support

Public benefit impact:
This program reduces barriers for Asian American seniors who often lack language-appropriate support and struggle to access available state-funded long-term care options.

2. Medi-Cal (Medicaid) & Medicare Long-Term Care Benefits Counseling

Many families are unsure how Medi-Cal (white card) and Medicare (red/blue card) apply to long-term care. AXH Foundation provides clear, accessible explanations of available benefits and how they connect to community-based or residential care settings.

We offer:

  • Medi-Cal LTC eligibility explanation

  • Medicare coverage overview (including post-hospital and chronic care benefits)

  • Guidance on choosing appropriate plans

  • Integration of benefits with ALW, RCFE, ADHC, and other long-term care services

3. Senior Care Navigation

Navigating the U.S. long-term care system is complex, especially for immigrant families. AXH Foundation serves as a culturally informed navigation center to help seniors and caregivers identify appropriate resources.

Care navigation may include:

  • RCFE / Assisted Living options

  • ADHC / CBAS (Adult Day Health Care)

  • Home Care and Home Health services

  • Skilled Nursing Facility (SNF) pathways

  • Community-based aging programs

  • Caregiver support resources

4. ECM & ILOS Care Coordination (CalAIM Support Services)

Under California’s CalAIM initiative, Enhanced Care Management (ECM) and In-Lieu of Services (ILOS) provide critical support for high-risk seniors with chronic conditions.

AXH Foundation provides non-clinical, culturally competent care coordination that helps stabilize health and improve quality of life.

Services include:

  • Comprehensive needs assessments

  • Individualized Care Plans (ICP)

  • Appointment and resource coordination

  • Chronic condition support

  • Home- and community-based services linkage

  • Ongoing follow-up and wellness monitoring

These services reduce preventable ER visits and hospitalizations, improve health outcomes, and address health disparities among immigrant seniors.

6. Asian Senior Research & Advocacy

AXH Foundation conducts and publishes research addressing the specific needs and disparities affecting Asian American seniors in California.

  • Health equity and aging outcomes
