AgeX Health Foundation is a nonprofit dedicated to strengthening long-term care for older adults and medically complex individuals.
We provide care coordination, caregiver support, and community-based health programs designed to close gaps in access and improve health equity.

Aging should not be a burden carried alone.
Our mission is to support aging adults — especially those with medical and social vulnerabilities — by strengthening care coordination, educating caregivers, and expanding community-based services that keep families connected and safe.
We believe every person deserves to age with dignity, compassion, and equal access to care — regardless of income, language, or background. AXH Foundation is dedicated to improving access, equity, and quality of care for Asian American seniors by providing navigation, education, benefit counseling, and culturally informed long-term care support.
